Wisconsin offers a variety of Medicaid programs for its residents, including BadgerCare Plus,
the Medicaid Purchase Plan (MAPP), Qualified Medicare Beneficiary (QMB), Specified Low-Income
Medicare Beneficiary (SLMB and SLMB+), Family Planning Only Services, Long-Term Care, Family
Care, and Wisconsin's Caretaker Supplement (CTS). These healthcare programs are designed to
support Wisconsinites with low incomes. Each Medicaid program has specific eligibility

There are four ways to apply for a Wisconsin Medicaid Program:
- Call the Northern IM Consortium at 888-794-5722. The hours are 8:00am-4:00pm, Monday through Wednesday, 11am-4pm on Thursdays, and 8am-2pm on Fridays.
- Apply online through Access
- Complete a paper application (under Medicaid: Forms and Publications)
- In-person at the Wood County Human Services Office at the River Block Building or Patriot’s Tower. Call 715-421-8600 to speak with the receptionist to arrange an appointment. Office hours are Monday through Friday 8:00am-4:30pm
